JodyJazz Trial Program Terms & Conditions Agreement Form

Thank you for your interest in the JodyJazz Trial Program.

Our Trial Program gives players the unique opportunity to compare multiple mouthpieces in the comfort of their own home before making a final decision. Because this program involves shipping multiple mouthpieces for evaluation, participation is subject to the Terms & Conditions outlined below.

Trial Program Summary(Required)
  • Purchase one mouthpiece and receive up to two additional mouthpieces for side-by-side comparison.
  • The trial period is limited to 3 weeks (21 days) from the shipment date.
  • Customers are responsible for returning all unwanted mouthpieces within the allowed timeframe.
  • Sanitization fees apply to all trial mouthpieces.
  • Expedited and International shipping charges are non-refundable.
  • Any returned mouthpiece showing visible wear or damage will be subject to a 15% refurbishment/restocking fee.
Sanitization Fees(Required)
  • $5 per mouthpiece under $300
  • $10 per mouthpiece priced at $300 and above
B-Stock Metal Mouthpieces(Required)

For metal mouthpiece trials, JodyJazz may send B-Stock mouthpieces whenever possible. B-Stock mouthpieces are in perfect playing condition but may contain minor cosmetic imperfections or finish blemishes. B-Stock mouthpieces are eligible for a 15% discount compared to standard retail pricing.

Important Care Guidelines(Required)

To avoid damage during your evaluation period, we strongly recommend avoiding:

  • Metal ligatures
  • Synthetic reeds
  • Extended or heavy playing
  • Placing mouthpieces on hard surfaces
  • Any handling that may cause scratches or cosmetic wear
Abuse of Trial Program Privileges(Required)

Customers who repeatedly request excessive trials or return excessively damaged mouthpieces may have their Trial Program privileges suspended at the discretion of JodyJazz Inc.

Trials are limited to no more than 3 Trial Program orders within a 12-month period per customer.

Agreement(Required)

By submitting this form, you acknowledge that you have read, understood, and agreed to the JodyJazz Trial Program Terms & Conditions. Submission of this form does not guarantee approval into the Trial Program. All trial requests remain subject to review and approval by JodyJazz Inc.
